Two dental plans are offered, an Exclusive Provider Organization, EPO Plan #9098 and a Delta Dental Preferred, DPO Plan #1563.  Click on the plan resources for detailed information.  Effective July 1, 2012 these plans will be on a fiscal year.  (July thru June)
 
EPO Plan 9098 is a copay for service plan.  You MUST use a PPO Provider for these services.  There is no annual maximum for this plan.
 
DPO Plan 1563 allows you to use any dental provider.  Deductible of $25 for individual and $50 for family.  Coverage is then at 80% on Basic Dentistry and 60% for Major Dentistry.  Annual Maximum paid by Delta, effective July 2012 is $2,000 per member.  This does not include orthodontics.
 
Dependent Children age 19 to 24, must be full-time students to be eligibly for coverage.  Class schedule is required as proof of student status prior to any claims payment.  You can fax your students schedule directly to Delta Dental to expedite eligibility requirements.  Verbal notification is needed in your Benefits Office to keep your student active.
